It's your NAT at home and at work that's causing the issues. The RTP stream after the call is setup always tries to go directly between IP addresses. So at your office your phones may be behind a firewall using 10.x.x.x and your house may be using 192.168.x.x, well the 10.x.x.x doesn't know how to route to 192.168.x.x directly.
Download and install the newer sipX 4.0 that supports remote workers behind a NAT. Then put your PBX behind your firewall and follow the instructions on how to configure the PBX and the remote users.
