Have you considered:

1) http://www.CallCentric.com: Unlimited incoming calls. Arnd 22 US$/Month for Pakistani DID
2) http://www.VoxBone.com: IP forwarding service only. 23 US$/Month for Pakistani DIDs
3) http://www.voiptalk.org: They started offering Pakistan DIDs for under 9 GBP/month, which was the cheapest I had seen anyhwere.... then suddenly, Pakistan got removed from their DID offering. I think they'll come back shortly with rates revised higher.

Voxbone has lowered their rates for Pakistan. It's $20/mo now!

Forget about PakistanTel.net, their customer support is ridiculous. It's the worst I've ever seen!

I like Net2Speech customer support. I filed a complaint for busyout and got support call with 24 hours from 2 engineers and got my new number, another phone call after 1 day. Its okay to have problem but its not okay to keep customers in dark. Fairly professional desis ;-))
