Default Cheap Voip Solutions?

Hello everyone, i know many people have searched far and wide to find a good, cheap, voipservice that allows byod. Well i could use a little help finding one myself. If you know of any that meets the following conditions please post it. Here's the conditions no more than 14.99 a month before taxes, extremely low or free activation fee must at least have unlimited U.S. calls. Oh it must have a Kokomo, IN numbers available. I thank you for any help that is given, Thank You!!
