Default Asterisk & iConnectHere

I have been trying to get this working for some time now.
  1. We have a US number and incoming calls works sometimes. A lot of people have experienced being sent straight to our voice mail even though there should be no reason for this. The easy fix to this has been asking people to call us right back if that happens. It sucks but it works.
  2. Outgoing calls don't work at all. If I set our phone number as username like I do in the register line I get a lady telling me: "Your account is temporarily unavailable .....". If I set it to my 8 digit login name I just get a busy tone.

I have tried both and countless variations of them but the result is either the lady or a busy signal. The busy signal is from asterisk because iConnectHere returns a "Bad Request" on my attempt to make the call.

Does anyone have any idea on what is going on and what I need to do? The machine is not behind NAT so sipauth should be the right host.
