Default Three way calling

Using SPA3102. PSTN line disconnected. I subscribed to a 416 unlimited in/out Toronto number from VBUZZER. I managed to do three way calling in a chain. I call a person on a land line and in turn he puts me on hold and calls a third land line person. This way we have a conference of three people.

It seems that I can not make a three way calling when I am the initiator of the two calls. I always get a busy signal from the second person. How many inbound and outbound channels are required to do three way calling when I am the initiator of the two calls?
