Call route/forward SIP solution

I am looking to implement following solution:
Receiving calls to SIP Server and then forward it to another landline or mobile number
I used the solution suggested here, But it does not seem to work

Can someone tell me what I need to change to make this work or what I am doing wrong

I have tried the following SIP address and all fail:
1) sip:hdsouza@proxy01.sipphone.com
2) hdsouza@proxy01.sipphone.com
3) sip:17473682738@proxy01.sipphone.com
4) 17473682738@proxy01.sipphone.com
5) 17473682738@gizmoproject.com
