What you are looking for is www.lingo.com As you request, for 19.95 a month, you get unlimited calls to Alaska, Hawaii, Puerto Rico, Canada, and as an extra bonus, you also get all of Western Europe ! How is that for a great deal ! I am certain that it is the only service that offers unlimited calls to all of these different and diverse places, as part of their basic package. And you also get a Free Month when you sign in, and if you purchase your own D'Link DVG-1402S/L VoiP Router & Adapter ( Free at New Egg after Rebate ) you can get service for $7.95 a month and not have to pay the $39.90 Pre-Payment or the $29.95 Activation Fee, or the $9.95 Shipping Fee to have them send you their own Lingo Module, and the $20.62 Subscriprion Charge; so in essence you will be saving a $100.42 investment, and the need to return the Lingo Telephone Adapter if for any reason you decide to quit the service, so as to not get charged the price of their telephone adapter. Broadvoice does not do forwarding to international numbers and Puerto Rico is $0.05 a minute!

Dialpad no longer mentions any sort of BYOD support although the previous/current configuration still works.

I may be atypical but that's a requirement for me.
