Default Troubleshooting Asterisk / SPA-3000 connection

I had it working for a while my connection [demo] my * would answer the PSTN line call but it stopped working.
The phone rings but * is not answering the call.

How to troubleshoot the connection?

2.) Is it possible include in SPA-3000 configuration wizard outside line dialing; ex. pressing 9 would give me an outside dial tone.

The default dial plan in the SPA-3000 Wizard already does this, though I use #9 instead of just 9.

I've used SPA-3000 configuration script and VOIP to FWD works OK
but "#9" to get and outside dial tone is not working:

I did not change anything in dial plan script, so my settings are:
([2-79]11<:@gw0>|xx.|*xx.|**xx.|<#1,:>xx.<:@gw1>|<#9,:>xx .<:@gw0>|<#9,:>*xx<:@gw0>)

My asterisk server is OFF so it is not interacting with SPA-3000
Why I have no dial tone when I press "#9"?

Another strange thing I've noticed when I upload the configured scrip to SPA-3000 that when an outside call comes from PSTN my pone Line 1 rings once, stops then after 13-15sec. it rings again stops; when I pick up the Line one I can not hear anything and the outside call is still ringing my line (I have two phoe lines so I'm just testing it).
